Your first appointment
Scheduling:
New patient appointments may be scheduled by calling our main number 512-454-9426.
A review of your medical records will need to be obtained prior to your first visit. We ask that you provide us with any records from your last 6 months by your primary or referring physician and any other physician who has treated your condition, including any MRI or CT reports.

What to bring:
After you make your first appointment you will be sent a comprehensive questionnaire (by mail, fax, or email). It is important for you to fill out the questionnaire as completely as possible before coming to your appointment; it will save time and also will help the nurse or physician understand your situation more clearly.
If you have your paperwork completed prior to your appointment, we ask that you arrive 15 minutes early. If you have not received your paperwork or have not completed the information, please arrive 45 minutes early.
Following is a list of items necessary for your first appointment:
- Consultation Packet
- Complete list of any medications you are currently taking
- Picture I.D.
- Insurance cards (if applicable)
What should established patients bring to their appointment?
- We ask that you bring your insurance card and list of current medications to every appointment
Where to go and park:
Central Office: Our Central Office is conveniently located on the fourth floor of Central Park Medical Building next to the Heart Hospital of Austin and Central Market at 38th Street and North Lamar, at 900 West 38th Street, Suite 400. There is paid parking in the garage behind the buildings. We do not validate parking. Ask about their senior discount.
Round Rock Office: Our Round Rock Office is conveniently located at 15808 FM 620 North, Suite 220. There is free parking in front and around the building.
